New Patient Information

Hyde Park Pediatrics is currently accepting newborns and children who are new to Greater Cincinnati or are transferring within our area as new patients. We accept most health insurance plans. Please call us with your insurance information. 

There are a few forms that you need to fill on your first visit. To save time, you can complete the forms in advance.  Print and complete the new patient forms (Registration form, Patient history, Financial form and Privacy Policy Acknowledgment) or just arrive a few minutes early and fill out the forms prior to your visit.

Families with Newborns

We would love to have the privilege of providing pediatric care to your newborn and family.  All of the pediatricians in our practice are accepting newborn/new patients. If you would like to meet one of our pediatricians, please call us to schedule a complimentary prenatal/new patient consultation. If you have identified Hyde Park Pediatrics as your primary care provider,  and you deliver at Bethesda, Christ or Good Samaritan Hospitals, the hospital will contact us automatically to let us know that your child has been born and one of our pediatricians will come to meet you and your baby, usually within 24 hours. If your child is born at any other hospital, a staff pediatrician will see your baby for their first exam.  Please call prior to leaving the hospital to schedule your newborn’s first visit with us. We like to see most newborns within 7 days of leaving the hospital. If you are breastfeeding, the first visit should be scheduled on a Wednesday in order to take advantage of the services of our Lactation Consultant. In addition, please call your insurance company within 7 days of your child’s birth to make sure your child is added to your family plan. We look forward to meeting you!

Transferring to Hyde Park Pediatrics

If you are planning to join Hyde Park Pediatrics, please contact your previous practice and sign a “transfer of records” form. It is important that we have the opportunity to review your child’s complete medical record prior to your first visit. This will ensure continuity of care and enables us to provide the best possible care to your child. You can request that your child’s records be mailed directly to us, or you can drop them off at our office. Once your child’s medical records have been received, we will be happy to schedule his or her first appointment. You may need to call our office once you have made your request to make sure your child’s records arrive in a timely manner. We will not schedule any routine appointments until the complete records arrive in our office.
